Supplements and Blood Pressure Medicines: What to Check Before You Add Arginine

Quick answer

Ask a pharmacist or doctor before combining arginine with blood pressure medicine, nitrates for chest pain or sildenafil-type drugs. Arginine can lower blood pressure, and stacking effects can push it too low. In studies it lowered pressure by about 5 mm Hg at 4 to 24 g a day; VapoCept lists 200 mg of L-arginine plus 200 mg of arginine AKG.

The short answer01

Can you take arginine with blood pressure medicine?

Not without asking first. Arginine can lower blood pressure, and so can the medicine you already take. Together the drop may be larger than either alone.

Mayo Clinic says arginine might lower blood pressure, so mixing it with blood pressure drugs, nitrates for chest pain or sildenafil (Viagra, Revatio) might drop pressure too low (Mayo Clinic, L-arginine). Memorial Sloan Kettering also flags blood pressure medicines (MSK, Arginine (About Herbs)).

Never swap medicine for a supplement

Keep taking what you are prescribed. A supplement is not a drug, and the FDA has not evaluated the statements made about VapoCept. Your pharmacist can check your whole list in a few minutes.

How it works02

Why does arginine lower blood pressure?

The proposed reason is that the body can turn arginine into nitric oxide, a signal that relaxes blood vessel walls. Relaxed vessels carry blood at lower pressure.

That is a proposed mechanism, studied mostly in blood pressure and exercise work. A meta-analysis of 11 trials (387 people) found 4 to 24 g of arginine a day cut systolic pressure by 5.39 mm Hg and diastolic by 2.66 mm Hg (Dong 2011, Oral L-arginine and blood pressure meta-analysis). Citrulline, another VapoCept line, showed systolic drops near 4 mm Hg at gram doses (Yang 2019, L-citrulline and blood pressure meta-analysis) (Citrulline and blood pressure meta-analysis (Avicenna J Phytomed)).

For someone with normal pressure, 5 mm Hg may mean little. For someone already on blood pressure drugs or a nitrate, it can be the extra push toward dizziness.

Label amount vs research amount, log scale

  1. L-Arginine (base)

5% of the low study amount

4,000 to 24,000 mg a day in blood pressure trials; 2,000 to 20,000 mg in exercise reviews (Dong 2011, Oral L-arginine and blood pressure meta-analysis)

  2. L-Arginine AKG extract

5% of the low study amount

4,000 to 24,000 mg a day, the same arginine range; the AKG share of the 200 mg is not stated (Dong 2011, Oral L-arginine and blood pressure meta-analysis)

  3. L-Citrulline HCl

1.7% of the low study amount

6,000 to 8,000 mg a day (blood pressure and exercise studies) (Citrulline, perceived exertion and soreness meta-analysis (2020))

  4. L-Citrulline Malate

1.7% of the low study amount

6,000 to 8,000 mg a day; 8,000 mg of citrulline malate is the usual exercise amount (Citrulline, perceived exertion and soreness meta-analysis (2020))

  5. Beta-Alanine

about 1% of the low study amount

4,000 to 6,000 mg a day for weeks in exercise studies (NIH ODS, Exercise and athletic performance (health professional))

  6. Niacin

    47% of the adult RDA

RDA 16 mg NE for men (14 for women); upper limit 35 mg a day from supplements (NIH ODS, Niacin (health professional))

  7. Calcium

    1.8% of the adult RDA

RDA 1,000 mg a day for adults 19 to 50 (NIH ODS, Calcium (health professional))

Cyan marker: the amount in the brand's panel, as listed. Graphite bar: the range used in research (amber bar for the RDA up to the upper limit). Each step to the right is ten times more. A studied dose says what was tested, not what works for you, and no study tested VapoCept.

The medicine list03

Which medicines need a check before arginine?

Five groups come up in the sources we cite. The list is not complete, so show your pharmacist everything you take, including vitamins.

Medicine groups to discuss before using arginine, with the reason and where it is stated
Medicine groupWhy it mattersSource
Blood pressure medicinesAdded lowering of pressureMayo and Memorial Sloan Kettering (Mayo Clinic, L-arginine) (MSK, Arginine (About Herbs))
Nitrates for chest painPressure might fall too lowMayo (Mayo Clinic, L-arginine)
Sildenafil-type medicinesPressure might fall too lowMayo (Mayo Clinic, L-arginine)
Blood thinners and antiplateletsA possible bleeding concernMayo (Mayo Clinic, L-arginine)
Recent heart attackArginine is not recommendedMayo, MSK and the VINTAGE MI trial (Mayo Clinic, L-arginine) (MSK, Arginine (About Herbs)) (VINTAGE MI trial, L-arginine after heart attack (ACC summary))

The nitrate, sildenafil and bleeding warnings come from Mayo only. We report them as cautions, not measured rates.

The heart attack warning04

Why is arginine not recommended after a heart attack?

In one trial of 153 heart attack survivors, 6 people died on about 9 g of arginine a day and none died on placebo. The trial stopped early (VINTAGE MI trial, L-arginine after heart attack (ACC summary)).

Patients started 3 to 21 days after the attack. The study was not about healthy adults, and 9 g is 45 times the 200 mg L-arginine line. Still, Mayo says arginine might raise the risk of death after a recent heart attack (Mayo Clinic, L-arginine), and MSK repeats the warning (MSK, Arginine (About Herbs)).

After a heart attack, arginine is a doctor's decision, even at a small label amount.

Where VapoCept fits05

What does this mean for VapoCept users?

VapoCept lists 200 mg of L-arginine and 200 mg of L-arginine AKG per capsule, plus two citrulline lines at 100 mg each (as listed in the brand's panel; check your bottle). The amounts are far below the studied doses.

Against the 4 g low end of the blood pressure meta-analysis (Dong 2011, Oral L-arginine and blood pressure meta-analysis), 400 mg of arginine would be 10 percent, if both arginine lines were counted as pure arginine. No study tested VapoCept, so any blood pressure effect at that amount is unknown. Label warnings: The label's warnings were not supplied.

The ingredient interacts on paper, so the question goes to a pharmacist first. See the L-arginine page and the side effects page for more.

1. Do you take a blood pressure medicine, a nitrate such as nitroglycerin, or an erectile dysfunction medicine such as sildenafil?
2. Do you take a blood thinner or a diabetes medicine, or have a bleeding disorder?
3. Do you have kidney disease, asthma, low blood pressure or a history of herpes, or have you had a heart attack?
4. Are you pregnant or nursing, or under 18?
5. Do you have sudden confusion, trouble speaking, a drooping face, or weakness on one side?

Before you start06

What should you ask your pharmacist or doctor?

Bring the bottle, or the panel from our supplement facts page, and ask three questions.

  1. Does anything I take lower blood pressure?

Name every prescription, over-the-counter drug and supplement.

  2. Should I check my pressure at home?

Ask whether to log readings for the first weeks, and what number should make you call.

  3. Is there a reason to skip this one?

Ask about a heart attack history, surgery plans, pregnancy, kidney or liver disease, and blood thinners.

The FDA does not approve supplements before sale or check that they work (FDA 101, Dietary supplements).

Warning signs07

What are the signs that pressure has dropped too far?

Dizziness on standing, light-headedness, a fast heartbeat or near-fainting can mean low blood pressure. Stop the supplement and call your clinician or pharmacist.

Chest pain, severe shortness of breath, fainting or one-sided weakness is an emergency. Call 911 and do not drive yourself.

Report a bad reaction to the FDA's reporting portal (FDA, Questions and answers on dietary supplements). For an order question, call +1 (507) 448-8190.

FAQ09

VapoCept questions people ask

Can I take arginine with blood pressure medicine?

Ask your pharmacist or doctor first. Mayo Clinic says arginine might lower blood pressure, so combining it with blood pressure drugs might push pressure too low. Keep taking your prescribed medicine either way.

Is arginine safe with nitroglycerin or other nitrates?

Mayo Clinic says combining arginine with nitrates for chest pain might make blood pressure too low. This is a caution from one source, so check with a pharmacist before taking any arginine product.

Does arginine interact with Viagra or Revatio?

Mayo Clinic lists sildenafil (Viagra, Revatio) as a possible concern, because both can lower pressure. Ask a pharmacist before combining them, even at the small amount VapoCept lists.

Should I take VapoCept after a heart attack?

Arginine is not recommended after a recent heart attack, according to Mayo and MSK. Ask your cardiologist first. The label amount is small, but the question belongs to your doctor.

Does VapoCept lower blood pressure?

We cannot say. No study tested VapoCept. Arginine lowered pressure at 4 to 24 g a day in a meta-analysis, and the label lists far less. Stay alert for dizziness and ask a pharmacist.

Do I stop my medicine if I start VapoCept?

No. Keep taking prescribed medicine as directed. A supplement is not a drug and does not replace one.
